Patients in their 40s, 50s, and beyond make up a steadily growing share of our orthodontic patients — and treatment works well at any of those ages. Still, it’s worth understanding what genuinely differs from treating a teenager so expectations are set correctly from the start.

Bone remodels more slowly. Teenagers have highly active bone turnover, part of why adolescent treatment tends to move quickly. Adult bone remodels too, just more gradually, which can mean a somewhat longer timeline for comparable tooth movement — not that it won’t work, just that patience matters more.

Gum and bone health needs a closer look first. By your 40s and beyond, prior dental work, wear, or mild gum recession are common, and solid periodontal support is a prerequisite for safely moving teeth. We coordinate with your general dentist or a periodontist when that foundation needs attention before treatment starts.

Existing dental work shapes the plan. Crowns, bridges, implants, and missing teeth all need to be accounted for differently than in a teenager’s plan, since they don’t respond to orthodontic force the way natural teeth do.

Bite and jaw issues tend to be more established. A narrow jaw or bite discrepancy that could have been addressed with simple growth guidance in childhood may now call for MARPE, MSE, or a combined surgical-orthodontic approach in a fully grown adult, depending on severity.

The upside: adult patients tend to be remarkably consistent with aligner wear and hygiene, which often offsets the slower biological pace. And because a large share of adult cases involve limited relapse correction rather than full comprehensive treatment, timelines are often shorter than people expect walking in.
